SEFB VOLUNTEER WAIVER, RELEASE & AGREEMENT

Please read this Volunteer Waiver, Release & Agreement carefully. By volunteering with Southeastern Food Bank and electronically acknowledging this agreement, you confirm that you have read, understood, and agree to the terms and conditions outlined below.
HEALTH & PARTICIPATION
In consideration for my ability to participate in volunteer activities with Southeastern Food Bank ("SEFB"), I acknowledge that volunteer activities may include standing, bending, sorting, packing, repetitive tasks, and lifting items weighing up to approximately 25 pounds. I certify that I am able to safely participate in volunteer activities and will notify SEFB of any accommodations or concerns that may affect my ability to participate safely.
SUPERVISION POLICY
I understand that volunteers must be able to participate independently and follow instructions in a warehouse environment. I acknowledge that SEFB does not provide personal care, caregiving services, custodial services, or one-on-one supervision. If I am under 18 years of age, or if I require supervision to participate safely, I understand that I must be accompanied by a parent, legal guardian, caregiver, or approved group leader who remains onsite during the entire volunteer shift.
POLICIES AND SAFETY RULES
For my safety and that of others, I agree to comply with all SEFB volunteer policies, safety rules, and directions for volunteer activities. I understand that closed-toe shoes are required while volunteering. I understand that volunteer activities may involve warehouse operations, moving equipment, lifting, repetitive tasks, and vehicle traffic in loading and parking areas. I further understand that volunteers may not operate forklifts, pallet jacks, warehouse equipment, or vehicles.
AWARENESS AND ASSUMPTION OF RISK
I understand that my volunteer activities may involve inherent risks that may arise from the activities themselves, SEFB operations, my own actions or inactions, or the actions or inactions of SEFB, its directors, officers, employees, agents, other volunteers, and others present at SEFB. These risks may include, but are not limited to, working around vehicles, warehouse operations, loading and unloading activities, moving equipment, lifting objects, performing repetitive tasks, slips, trips, falls, bodily injury, death, and property damage. I assume full responsibility for any and all risks of bodily injury, death, or property damage caused by or arising directly or indirectly from my presence at SEFB program sites or participation in SEFB activities, regardless of the cause.
WAIVER AND RELEASE OF CLAIMS
I waive and release any and all claims against SEFB, its directors, officers, employees, volunteers, affiliates, and agents (collectively, the "Released Parties"), for any liability, loss, damages, claims, expenses, and attorneys' fees resulting from death, injury to my person, or damage to my property caused by or arising directly or indirectly from my presence at SEFB or participation in activities on behalf of SEFB, regardless of the cause and even if caused by negligence, whether passive or active. I agree not to sue any of the Released Parties on the basis of these waived and released claims. I understand that SEFB would not permit me to volunteer without my agreement to these waivers and releases.
EMERGENCY MEDICAL CONSENT
In the event of an illness, injury, or emergency, I authorize SEFB to contact emergency medical services and the emergency contact provided by me. I understand that SEFB does not provide medical personnel or medical care. I waive and release any claims against the Released Parties arising out of efforts made in good faith to obtain emergency assistance on my behalf, including the timing or manner in which such assistance is requested.
INDEMNIFICATION
I agree to hold harmless and indemnify SEFB, its directors, officers, employees, volunteers, affiliates, and agents from any claims, damages, losses, expenses, or attorneys' fees arising from my volunteer activities.
COMMUNITY SERVICE POLICY
I understand that Southeastern Food Bank does not provide verification, tracking, or signatures for court-ordered, probationary, or legally mandated service requirements.
PHOTO & VIDEO RELEASE
I consent to the unrestricted use of my image, voice, name, and/or story in any format, including video, print, or electronic media, that SEFB may create in connection with my participation in volunteer activities. I understand that such materials may be used on websites, social media platforms, publications, marketing materials, and other promotional materials. I waive any right to inspect or approve the finished product and acknowledge that I am not entitled to compensation for the creation or use of such materials.
CONFIDENTIALITY
I understand that, as a volunteer, I may have access to sensitive or confidential information relating to SEFB clients, volunteers, donors, staff, or operations. I agree to maintain the confidentiality of such information and not disclose or use it except as required in connection with my volunteer activities or as authorized by SEFB.
VOLUNTEER NOT AN EMPLOYEE
I understand that I am not an employee of SEFB, that I will not be paid for my participation, and that I am not covered by or eligible for workers' compensation, health insurance, employee benefits, or other employment-related benefits through SEFB. I understand that I may choose at any time not to participate in an activity or to discontinue my volunteer service.
